Dr. Dardamanis (Γιατρος Δαρδαμανης) expalians with simple words what is Mini gastric bypass.
What is Mini Gastric Bypass (MGB) – (MGB)
The mini gastric bypass is a variation of the classic bypass, which includes only one anastomosis, ie an intestinal suture with the omega-shaped stomach (while in the classic bypass we have two anastomoses, one with the stomach and one with the intestine in the shape of an ipsilon. In any case, the purpose is to bypass a significant part of the small intestine that will not absorb nutrients (and fat!) And to reduce – bypass the larger volume of the stomach (reduce food intake).
Therefore, it aims at the reduced intake but also at the reduced absorption of the food intake.
Faster satiety and fewer calories
When to do a mini bypass
Like all weight loss surgeries, failed weight loss efforts through diet or other conservative measures must be preceded by specific criteria for weight and comorbidities. Surgical treatment is recommended in morbid obesity
Body Mass Index (BMI), ie the quotient of weight in kilograms to height in meters squared, 40 kg / m2 or more Body Mass Index between 35 and 40 kg / m2, when there is a disease associated with obesity, such as type II diabetes, hypertension, hyperlipidemia, sleep apnea, etc.
How the operation is performed
Pre-operational check
- It is preceded by the classic preoperative examination and definitely a gastroscopy, examination for H. pylori and stomach biopsies.
- Electrocardiogram and cardiological examination
- Chest x-ray and liver ultrasound
- General blood and coagulation test as well as special tests for some trace elements and vitamins.
How the surgery is done
The surgery is performed laparoscopically (ie with small holes in the abdominal wall and with the help of special tools and a laparoscopic camera.
Total anesthesia is performed
A new, smaller stomach (new stomach) is created, then with the esophagus receiving food, the rest and most of the stomach is isolated from the process of digestion (bypassed). The new stomach is joined with special stapling tools with a part of the small intestine about 2 meters lower. Thus, the food passes from the esophagus to the smaller new stomach and from there directly to the peripheral small intestine, where it will meet bile and pancreatic enzymes, and the process of absorption will begin, but 2 meters later!
Advantages of gastric bypass
- Laparoscopic access is made with small and flawless cosmetic incisions
- Has minimal postoperative pain
- Infections and postoperative hernias are eliminated
- The hospital stay is 2-3 days
- Immediate return to daily activities and work
- It works in a double way, that is, by reducing the food intake due to a small stomach but also with reduced absorption from the bypass of 2 meters of small intestine.
Postoperative care
- After 2 or 3 days of hospitalization, a special diet is followed which is mainly in liquid or semi-liquid form (puree, cream) with supplements of proteins and vitamins.
- No special medication is required
- No special wound changes are needed (tiny incisions – no sutures cut)
- Immediate recovery and physical activity.
Results – Weight loss
- You lose about 70-80% of your above weight
- You lose most pounds in the first half after surgery
- The weight loss continues for another year and a half
- After 2 years in total, the weight loss is complete and you stabilize in your new kilos, without gaining them again.
With proper nutrition and the addition of light physical activity, you lose all the extra pounds and return to your ideal weight and normal BMI.
